Acetaminophen (Tylenol) is one of the most commonly given pediatric medications, and also one of the most common causes of accidental drug overdose in children — almost always from stacking too many doses too close together, not from a single large dose. This calculator computes a standard weight-based dose and, just as importantly, enforces the two daily ceilings that actually prevent overdose.

How the calculator works

The per-dose amount is body weight in kilograms multiplied by a dose-per-kilogram figure between 10 and 15 mg/kg, matching FDA and AAP-cited pediatric dosing. That per-dose amount is converted to milliliters using the suspension concentration, which defaults to 160 mg per 5 mL — the single standardized concentration used across infant and children's acetaminophen liquid since 2011.

The calculator then checks that schedule against two independent daily ceilings and always reports the lower one as the safe maximum: a hard 5-doses-per-24-hours limit (what OTC package labeling actually enforces), and a 75 mg/kg/day cap that never exceeds an absolute 4,000 mg/day regardless of how large the child is.

Inputs and what they mean

Body weight (in pounds or kilograms) and age (in months) drive every other number. Age matters because OTC dosing charts, and this calculator, do not cover children under 24 months — infants that young need a pediatrician's guidance rather than a standard chart. The mg/kg slider and dosing interval let you match your product's label or a clinician's instructions, and the concentration field lets you match your specific suspension if it differs from the standard 160 mg/5 mL.

Limits and edge cases

This is an educational reference tool, not a substitute for clinical judgment — always confirm the actual dose with a pharmacist or the current prescribing information before giving any medication. It does not account for liver or kidney impairment, other medications containing acetaminophen (a common cause of unintentional double-dosing), or non-standard concentrations without you entering them manually. If a computed schedule is flagged as exceeding the safe daily maximum, reduce the per-dose amount or the number of doses per day rather than giving the dose anyway.
