Arterial (or vascular) age reframes cardiovascular risk as an age instead of a percentage — a way of saying 'your combined risk factors look like those of a person this many years older than you, if that older person had ideal levels of everything else.' This calculator builds that estimate from a published, point-based cardiovascular risk model. This article explains how the score is built, how the age conversion works, and where its limits lie.
How the underlying risk score works
The point tables come from the D'Agostino Framingham General Cardiovascular Risk Profile (Circulation, 2008), a widely used, sex-specific model that converts age, total and HDL cholesterol, systolic blood pressure (with treatment status), diabetes, and smoking into whole-number points. Some factors add points (higher cholesterol, higher blood pressure, diabetes, smoking); others subtract points (higher HDL, lower blood pressure). The points sum to a total that also maps to a published 10-year cardiovascular risk percentage, shown alongside the arterial age on the vs Actual Age tab.
How arterial age is derived from the risk score
Arterial age uses the iso-risk age-equivalence method: your total points are compared against a same-sex reference person who has every non-age factor at an optimal level (cholesterol under 160 mg/dL, HDL 60 mg/dL or higher, blood pressure under 120 mmHg untreated, no diabetes, no smoking). Subtracting that reference isolates an 'age-equivalent' point value, which is then mapped back onto the age axis using the model's own age-bracket point table.
Because so few real people meet every optimal criterion at once, arterial age estimates from this method commonly run higher than chronological age even for people who consider themselves healthy — this is a well-documented property of Framingham-based vascular age tools, not a sign that something is wrong with your numbers. When several risk factors combine, the estimate can exceed the model's oldest defined age bracket; the calculator reports '85+' in that case rather than an artificially precise number.
Limits and how it should be used
Arterial age is a communication tool, not a diagnosis or a life-expectancy prediction. It compresses several risk factors into one number, so two people with the same arterial age can have very different underlying profiles. The age-bracket system is coarse — everyone within a five-year age band, or in the open-ended '75 and older' bracket, is treated identically for the age component.
