The biophysical profile (BPP) is an antepartum test that combines four ultrasound observations of fetal activity with the non-stress test to assess fetal wellbeing. Developed by Frank Manning and colleagues in 1980, it remains a standard antenatal surveillance tool for pregnancies at increased risk of chronic hypoxia. This article explains how the score is built, how to read it, and where its limits lie.

How the biophysical profile works

Five components are each graded strictly 0 or 2 points — there is no partial credit — and summed to a total of 0 to 10. Four components (fetal breathing, gross body movement, fetal tone, and amniotic fluid volume) are assessed by real-time ultrasound over roughly a 30-minute observation window; the fifth, the non-stress test, is assessed by fetal heart rate monitoring.

The underlying logic is that acute markers of fetal central nervous system function (movement, tone, breathing, heart rate reactivity) are lost in a predictable sequence as hypoxia worsens, while amniotic fluid volume reflects more chronic uteroplacental function. Combining an acute and a chronic marker is what makes the BPP more informative than any single test alone.

Reading the score and interpretation

A total of 8 to 10 is generally normal and associated with a low risk of chronic fetal compromise. Because every component is worth 2 points, a single abnormal finding with everything else normal only costs 2 points and still lands at 8 — still normal. A nonreactive NST alone, for instance, is a common and often reassuring finding (it can simply reflect a fetal sleep cycle) and does not by itself pull the score out of the normal range. A score of 6 is equivocal and typically reflects two abnormal findings together — commonly a nonreactive NST paired with reduced amniotic fluid or reduced activity — and is commonly followed by repeat testing within 24 hours rather than immediate action. A score of 4 or below raises real suspicion for chronic fetal asphyxia, and Manning's original protocol further stratifies management by gestational age (for example, favoring delivery at or beyond 32 weeks and same-day re-testing at earlier gestations).

Amniotic fluid volume deserves special attention: an isolated abnormal AFV with an otherwise normal score is still a trigger for further evaluation, since chronic oligohydramnios can signal uteroplacental insufficiency even when the fetus's acute activity looks reassuring.

Limits and how it should be used

The BPP is a screening and surveillance tool, not a diagnosis. Many practices now use a modified BPP (NST plus amniotic fluid volume only) for routine surveillance, reserving the full 5-component test for when the modified version is abnormal or activity appears reduced on ultrasound — this calculator implements the full 5-component version, not the modified one.
