The cholesterol ratio, or TC/HDL ratio, is a simple screening figure that combines total cholesterol and HDL cholesterol from the same lipid panel into one number. It's widely cited in consumer cardiology education as a quick way to see how a person's "good" cholesterol stacks up against their total cholesterol, rather than looking at total cholesterol in isolation.
How the Cholesterol Ratio Calculator works
The calculator divides total cholesterol by HDL cholesterol, both entered from the same lipid panel and the same unit. Because it's a straight division of two same-unit values, the ratio itself carries no units — a ratio of 4.0 means total cholesterol is four times HDL, regardless of whether the underlying numbers were in mg/dL or mmol/L.
The three screening bands used here — Optimal (below 3.5), Desirable (3.5 to just under 5.0), and High risk (5.0 and above) — are drawn from cholesterol-ratio guidance commonly cited in patient-facing cardiology education, tracing back to lipid-profile risk research such as the Framingham Heart Study. Different sources sometimes draw the band edges slightly differently, which is why this calculator treats them as a general reference rather than a single fixed rule.
Inputs and what they mean
Total cholesterol and HDL cholesterol should both come from the same lipid panel lab result, entered in the same unit — mg/dL (the US convention) or mmol/L (used in most other countries), toggled with the unit switch. If your lab reports one value in mg/dL and you convert only one of the two inputs, the ratio will be wrong, so keep both fields in the same unit.
HDL is the input that most often surprises people: because it sits in the denominator, a small change in HDL can shift the ratio more than an equally sized change in total cholesterol. Two people with identical total cholesterol can land in very different risk bands purely based on their HDL level, which is part of why the ratio is considered more informative than total cholesterol alone.
Limits and edge cases
The TC/HDL ratio does not account for LDL particle size, triglycerides, apolipoprotein levels, blood pressure, smoking status, diabetes, family history, or any of the other factors that go into a full cardiovascular risk assessment (such as a Pooled Cohort Equations / ASCVD risk score). It is a quick screening heuristic, not a replacement for a comprehensive risk calculation or a clinician's assessment of the whole lipid panel.
An HDL of zero (or a missing HDL) makes the ratio mathematically undefined, so the calculator will not compute a result until a valid, non-zero HDL is entered. If your result looks unexpected, double-check that both total cholesterol and HDL are in the same unit and both actually come from the same blood draw.
