Total serum calcium readings are influenced by how much albumin is in the blood, because roughly 40% of calcium circulates bound to albumin. When albumin is abnormal — most often low, as in critical illness, malnutrition, or liver disease — the raw total calcium can misrepresent a patient's true calcium status. This calculator applies the standard Payne correction to estimate what total calcium would read if albumin were normal.

How the corrected calcium formula works

The formula is Corrected Ca = Total Ca + 0.8 x (4.0 - Albumin), where total calcium and the result are in mg/dL and albumin is in g/dL. Payne RB, Little AJ, Williams RB, and Milner JR first described this linear correction in a 1973 paper published in the British Medical Journal, based on the observation that total calcium falls by roughly 0.8 mg/dL for every 1 g/dL drop in albumin below the reference value of 4.0 g/dL.

The correction essentially adds back the calcium that would be bound to albumin if albumin were at a normal level, producing an estimate of total calcium that is less distorted by protein-binding effects.

Inputs and what they mean

Total calcium is the raw calcium value from a basic metabolic or comprehensive metabolic panel, in mg/dL. Albumin is the serum albumin level, in g/dL, ideally drawn from the same blood sample.

The lower the albumin falls below 4.0 g/dL, the larger the upward correction applied to total calcium. Conversely, if albumin happens to be above 4.0 g/dL, the correction lowers the calcium estimate slightly. When albumin is exactly 4.0 g/dL, the corrected and raw values are identical.

Limits and edge cases

Corrected calcium is an estimate, not a direct measurement, and its accuracy depends on the linear albumin relationship holding true. That relationship can break down in critically ill patients, in acid-base disturbances, and in chronic kidney disease, where protein binding and calcium handling are altered in ways the simple formula does not capture. In these situations, or whenever the clinical picture does not match the corrected value, order an ionized calcium — measured directly and unaffected by albumin — instead of relying on the correction. A single corrected calcium reading should always be interpreted alongside the full clinical picture, not as a standalone diagnosis.
