After 12 months of dual antiplatelet therapy (DAPT) following coronary stenting, clinicians and patients face a decision: stop therapy at 12 months, or continue it for an extended period. The DAPT score, derived from the DAPT Study (Yeh et al., JAMA 2016), quantifies which way that tradeoff likely leans for a given patient by combining nine clinical variables into a single number.
How the DAPT score works
The DAPT score sums a tiered age term with eight binary risk factors. Age is the only variable that subtracts points — patients 65 to 74 lose 1 point, and patients 75 or older lose 2 points, relative to patients under 65. The remaining eight factors (current smoking, diabetes, MI at presentation, prior PCI or MI, stent diameter under 3 mm, a paclitaxel-eluting stent, CHF or LVEF under 30%, and saphenous vein graft PCI) each add 1 or 2 points when present. The resulting total ranges from -2 to 10.
The score was derived by comparing outcomes in patients randomized to continued versus stopped DAPT at the 12-month mark, identifying which combination of baseline characteristics predicted a net ischemic benefit (fewer stent thromboses and heart attacks) versus a net bleeding harm from continuing therapy.
Reading the threshold
A score of 2 or higher identifies patients in whom the derivation cohort showed the ischemic benefit of continuing DAPT generally outweighed the added bleeding risk — supporting a shared decision to extend therapy, commonly to 18 or 30 months total, in appropriate candidates. A score below 2 identifies patients in whom bleeding risk generally predominated, favoring standard-duration therapy of about 12 months.
The score is intended to be read as this binary threshold rather than as a continuous risk percentage — it does not get progressively "more favorable" as the number climbs past 2, it simply crosses from one recommendation zone into the other.
Limits and how it should be used
The DAPT score applies only to patients who have already completed 12 months of DAPT without a major ischemic or bleeding event — it is not a tool for setting the initial planned DAPT duration at the time of stenting (that role belongs to scores like PRECISE-DAPT). It was derived in a specific trial population and stent-era context, so it should be applied alongside, not instead of, an individualized assessment of the patient's actual bleeding history, comorbidities, and preferences.
