Morphine milligram equivalents let clinicians and pharmacists compare, and sum, opioid regimens that mix several different drugs. A single daily MME number underlies the CDC's opioid-prescribing risk thresholds and is widely used in overdose-risk screening, insurance utilization review, and clinical chart review.

How MME is calculated

Each opioid a patient takes is converted to a morphine-equivalent dose by multiplying the dose taken per administration by how many times per day it is taken, then by that drug's CDC-published conversion factor. The results for every opioid in the regimen are added together into one total daily MME figure. A fentanyl transdermal patch is a special case: because it delivers continuously rather than in discrete doses, only the patch strength (in mcg/hr) is multiplied by its conversion factor — there is no frequency term.

Why CDC set 50 and 90 MME/day as reference thresholds

The CDC's original 2016 opioid-prescribing guideline treated 90 MME/day as close to a hard ceiling. The 2022 update, published in the Morbidity and Mortality Weekly Report, deliberately softened this: it kept 50 and 90 MME/day as reference points but reframed them as advisory bands rather than rigid limits, explicitly warning against misapplication as inflexible cutoffs that could lead to involuntary dose tapering or patient abandonment.

Under the 2022 language, clinicians should use caution when prescribing opioids at any dosage, carefully reassess the evidence of individual benefits and risks when increasing a patient's dose to 50 MME/day or more, and avoid increasing to 90 MME/day or more — or carefully justify and closely monitor a decision to do so. Overdose risk rises with dose, but the guideline is explicit that these are population-level reference bands, not individualized prescribing rules.

What this tool does not do

This calculator answers one question: given a current regimen, what is the total daily MME, and which CDC risk band does it fall into? It is a screening and education tool, not a substitute for clinical judgment, and it must never be used as the sole basis for a prescribing decision. It also does not perform opioid rotation — converting a dose of one opioid into an equivalent dose of a different opioid for a drug switch requires an equianalgesic table and a mandatory cross-tolerance dose reduction, which is a separate calculation with its own safety rules. Buprenorphine is intentionally excluded, since CDC's own conversion table assigns it no MME factor. Methadone uses CDC's single published factor of 3; in practice, some institutions use a dose-dependent sliding scale for methadone, since a flat factor can underestimate MME at high doses.
