A Body Shape Index is a body-shape metric that combines your waist circumference, height, and BMI to estimate abdominal-obesity-related risk that BMI on its own cannot see. Introduced in 2012 research linking it to mortality hazard, ABSI is designed to be almost independent of BMI, height, and weight — so it captures a genuinely different signal: how much of your waist size is "extra" once your overall size is accounted for. How ABSI works
ABSI divides waist circumference by BMI raised to the two-thirds power and by the square root of height. That specific combination of exponents was chosen mathematically so that, across a population, ABSI ends up almost uncorrelated with BMI, height, and weight themselves. In practice, that means a bigger BMI or a taller frame does not automatically push ABSI up or down — the number moves almost entirely with waist size.
That property is what makes ABSI useful as a complement to BMI rather than a replacement for it. BMI tells you about overall size; ABSI tells you, independently of that size, whether your waist is proportionally larger or smaller than typical.
Why waist shape matters beyond total weight
Fat stored around the abdomen — visceral fat — behaves differently from fat stored elsewhere on the body. It sits close to the liver and other organs, releases inflammatory signals, and is more strongly linked to cardiovascular disease, insulin resistance, and type 2 diabetes than total body fat. Two people can share an identical BMI and still carry very different amounts of visceral fat, simply because their weight is distributed differently.
BMI, using only weight and height, cannot see that distribution. ABSI reads the waist directly (while still accounting for BMI and height), so it can flag the abdominal-shape signal that BMI misses entirely.
ABSI vs BMI vs waist-to-height ratio
Waist-to-height ratio is the simplest waist-based screen — just divide waist by height. ABSI is more deliberately engineered: its formula specifically removes the BMI and height correlation that a raw waist measurement still carries, so its association with health outcomes in the original research was independent of BMI rather than overlapping with it. BMI remains useful as a first-pass overall-size screen, especially for flagging underweight, but it cannot see fat distribution on its own.
The strongest approach is to use them together: check BMI first, then look at ABSI (or waist-to-height ratio) to understand whether your body shape adds risk beyond what BMI alone suggests.
Limits and how to use the number
ABSI is a screening estimate, not a diagnosis, and it is for informational purposes only — it is not medical advice and cannot replace assessment by a qualified healthcare professional. It does not directly measure body fat or visceral fat; only imaging such as DEXA or MRI does that. Its z-score comparison in this calculator uses a single, sex-neutral approximate adult reference rather than the original research's full age- and sex-specific tables, so treat the band as a rough population comparison rather than a personalised percentile.
